MEDication Focused Outpatient Care for Underutilization of Secondary Prevention (MED-FOCUS) The goals of this study are to conduct a multi-center, cluster-randomized implementation study utilizing a web-based, centralized CVRS run by clinical pharmacists to support medical offices with large geographic, racial and ethnic diversity to determine the extent to which the CVRS model will be adopted and implemented.
