Dr. Moczygemba and her team are working to test the usefulness of Smartphone technology as a way for people to stay connected with health care services and to prevent the need to visit the emergency room.
- The purpose of this study is to test how well GPS technology on a Smartphone works to notify healthcare providers of a visit to the emergency room or hospital.
- We also want to know if the GPS technology and Smartphone helps to coordinate healthcare after an emergency room or hospital visit.
- The results of this study will be used to help use technology to coordinate healthcare in the future for a greater number of people and for a longer period of time.

The GPS technology on the Smartphone will allow a community paramedic to contact the participant and identify any needs he/she may have related to the ED visit or hospitalization.
