Collaborative Care Overview

The Collaborative Care model is an integrated care model that can be used in primary care and specialty settings.

The model centers itself around a Behavioral Health Care Manager. This individual works closely with the patient, primary care provider, and psychiatrist to closely monitor the patients progress through short term interventions used to treat mild-moderate behavioral health conditions.

The American Psychiatric Association has created a wonderful video that provides an overview of the model.
