To refer a student to TCHATT services, please complete the following steps:

Can’t access Trayt? Use the Alternative Referral Form.
Need guidance on referring a student in Trayt? Follow the steps below.
- Log in with your school email address and the personal password you created when you set up your account.
- On the “Referrals” page, click the “+ New Referral” button.
- Complete all fields on the referral form, including those that do not have an asterisk.
- Book an appointment in Step 2 afterwards.

If more than one guardian has medical decision-making rights for the student, and they aren’t a married couple, both guardians are required to attend the registration appointment. If one guardian cannot attend, they must complete the registration survey/enrollment packet before the appointment.

Updated Registration Process
Dell Medical School’s TCHATT program is adjusting its registration process to connect students to care sooner. These changes include:
- Students are required to attend registration appointments. Please keep this in mind when selecting an appointment time.
- Students will discuss goals for their care and complete necessary assessments with a clinician.
- We have transitioned to a new English REDCap registration survey to streamline the enrollment process. This survey walks through sections to provide required information, including consents for treatment and releases of information. Guardians can save their progress and return to complete the survey later if needed.
- This is a non-unique survey link, and campus liaisons can forward it as needed to guardians as a replacement for the English PDF enrollment packet.
- Guardians are encouraged to complete the survey before the registration appointment, but appointments can proceed even if the survey is incomplete. For Spanish-speaking families, the Spanish enrollment packet (PDF) will continue to be used until the REDCap survey is available in Spanish.
What is a registration appointment?
Registration appointments allow TCHATT staff to collect all necessary forms and information to determine the services the student will receive.
During this appointment, participants will be able to:
- Learn about TCHATT’s purpose and functions.
- Review participation and engagement requirements.
- Complete any outstanding sections of the enrollment process.
- Schedule an initial appointment.

Cancellation policy: To cancel or reschedule an appointment, please notify TCHATT at least 24 hours before the appointment. Advanced notice allows us to accommodate other students in need.
