Care Navigation
- Care navigation is a service delivered by a Care Navigator who seeks to address the wellness needs of clients through education and advocacy in navigating systems and mental health services. Care navigation connects clients to resources in a variety of areas, such as healthcare, housing, and finances, and works alongside them to anticipate and address barriers. Average duration is 6 months; 7 appointments.
- Locations: Services are delivered virtually, at the Amplify clinic located in Austin, Texas (Travis County), and in the community in any of the following service areas: Bastrop, Blanco, Caldwell, Hays, Travis, and Williamson
Employment and Education Support
- Individual Placements and Supports (IPS) is provided by a Supportive Employment and Education Coach (SEEC) who works with young people to engage them in career exploration, identify goals related to work and/or school, conduct skill building around work and school, and help the young person obtain and maintain meaningful employment and/or enrollment in appropriate educational opportunities. Average duration is 5 months; 8 appointments.
- Locations: Services are delivered virtually, at the Amplify clinic located in Austin, Texas (Travis County), and in the community in any of the following service areas: Bastrop, Blanco, Caldwell, Hays, Travis, and Williamson.
Wellness/Skill Building Groups
- Groups will include topics such as transitioning to independence, navigating systems of care, financial literacy, mindfulness, sexual wellness, and substance use prevention. Average duration is 5 weeks; 5 meetings for 1 hour each.
- Locations: Groups may be conducted virtually and at the Amplify clinic located in Austin, Texas (Travis County).

- Young adults attending community college who are experiencing mental health stresses, have higher social determinants of health needs, and are more likely to disengage from supports at this time in their lives.
