The Housing Authority of the City of Austin (HACA) is excited to announce the launch of The Health Catalyst Pilot Program, supported by a 2-year grant awarded by the St. David’s Foundation. The program is slated to commence in early 2019 at one HACA community. The primary program objectives are to serve residents at Booker T. Washington Terraces by conducting thorough individual needs assessments, developing custom Wellness Plans that link resident needs to existing local medical and social services, administer monthly walking groups, and execute culturally-responsive monthly health group visits in partnership with local health experts.
HACA seeks a dynamic, experienced and motivated self-starter to fill the Community Health Catalyst position, a full-time role responsible for successfully implementing HACA’s Health Catalyst Pilot Program. This position manages a team of two full-time Community Health Workers (HACA residents in-training). Position performs all activities associated with the success of the pilot program including project management, ensuring a creative and equitable resident engagement process, inspiring and protecting resident trust, follow up of referral service usage and effectiveness, relationship building and cross-collaborations with local health and social service providers, just to name a few.
