Caregiver/ Home Health Aide/Certified Nurses Aide at Spring Hills Home
QUALIFICATIONS:
- Must have at least one year of certifiable experience in home care, private-duty home care setting, community health agency, hospital or med-surg clinical rotation in a nursing school or have at least one year of verifiable work experience in an unrelated field and if in an employed state where necessary, have successfully completed a Home Health Aide Training course approved by the Vice President of Home Care.
- Demonstrate an ability to effectively communicate in English, and/or the predominate language of the population being services and must be able to read and write.
- Pass the Spring Hills Home Care Services Competency Evaluation Examination.
- Demonstrates competency in performing all skills.
- Pass a criminal background check.
- Meet all medical requirements
- Meet all state requirements.
- Have reliable transportation and method of communication
- Is punctual, dependable and demonstrates sound judgment.
JOB RESPONSIBILITIES:
- Follows the established Plan of Care.
- Assists with “Activities of daily living” means the functions or tasks for self-care which are performed either independently or with supervision or assistance. Activities of daily living include mobility, transferring, walking, grooming, bathing, dressing, undressing, eating, and toileting, per the Plan of Care.
- Assists with “Independent Activities of daily living” which may include light housekeeping, shopping, laundry and meal preparation, per the Plan of Care.
- Accompany clients to appointments (transportation is client’s responsibility unless driving waiver is signed and per the Plan of Care)
- Provides assistance with self-administered, pre drawn and medication reminders, per Plan of Care.
- Understands and maintains infection control and safety measures
- Reports any and all changes in the client’s condition to the Care Manager or BOM.
- Completes the Activity Records and accurately documents the care provided
- Performs all activities as directed by the Care Manager or BOM
- Has a positive relationship with clients and families
- Has positive client satisfactions
- Meets all deadlines for health screening requirements and training.
- Encourage maximum independence to facilitate rehabilitation as determined EP and POC.
- Respect client’s rights and ensure confidentiality
