ROLE AND RESPONSIBILITIES
- Perform initial assessments in clients’ place of residence to determine care needs and develop appropriate plans of care for families in need.
- Revises the plan of care as necessary to ensure quality and continuity of care.
- Present a professional appearance and demeanor when visiting new and existing clients to ensure high rate of conversion from prospect to client.
- Maximize sales potential by building a strong rapport with families, developing a thorough understanding of customer needs/requirements, and providing recommendations on the best arrangement of services (nursing, and/or care management).
- Ensure successful onboarding of new clients (e.g. new client paperwork) and establish care advising team as primary team responsible for day-to-day oversight of in-home care services.
- Establish transparency with ongoing pricing and cost of services prior to completing the admission.
- Serve as “account executive” for assigned clientele, maximizing the quality of care provided and profitability of services provided.
- Provides effective communication to client, family members, team members and other health care professionals
- Ensure and comply with all required protocols and documentation for various payors (i.e. Long-Term Care Insurance and Workers Compensation).
- Ensure that the system is updated with appropriate documentation of all client communication (including all in-home follow-up visits and phone calls) and client records (e.g. plan of care) in a timely manner.
- Collaborate with care coordination team to ensure that Care Providers placed in clients’ homes are the best fit for meeting the needs and requirements of families served.
- Coordinates services and informs physician and other agencies of clients condition and change of needs.
- Provide on-site training and education to Care Providers as needed and ensure that caregivers are always introduced to clients on or before their first day in a clients’ residence.
- Ensure that caregivers are educated on all relative signs and symptoms of a client’s diagnosis.
- Supervises, teaches and provides clinical direction to other nursing personnel. Supervises LPNs/LVNs and care providers providing services to our clients. May only conduct care provider competency evaluations if qualified with two years of clinical experience and one year of home health experience.
- Obtains physicians orders as necessary for admissions and ongoing clinical care.
- Track various quality metrics including, but not limited to readmission rates for clients, diagnosis-related recovery trends, change in condition, hospitalizations, discharges, etc.
- Uses infection control measures that protect both the staff and client (OSHA)
- Promote the agencies’ philosophy and administrative policies
- Performs on call responsibilities and provides on call services to clients and families as assigned
- Perform other reasonably related duties or cross-cover as needed or as assigned by the immediate supervisor/management level personnel.
- Fluent in basic Microsoft Office functions (Word, Excel, PowerPoint).
- Able to organize and maintain electronic client records.

QUALIFICATIONS AND EDUCATION REQUIREMENTS
- RN required.
- Strong candidates have minimum of 1 year of experience working in the home care industry (or related industry – hospice, home health, hospital, etc.)
OTHER REQUIREMENTS
- Ability to drive when required per company policy.
- Prolonged periods of sitting, standing and driving may be required.
- Ability to work on call per company policy.
